Looking for a Russian and English speaking service coordinator for Adult Day Health care center located in Needham, MA. The position requires working with eastern European and Hispanic community. Service Coordinator duties include working with participants to establish treatment goals in accordance with the individual plan of care under the supervision of a licensed Social Worker. Informing and referring participants to available community services. Writing service coordination notes for participants upon admission and at least once every 90 days or more if necessary.
